Most of our support for people with learning disabilities and autism is delivered through supported living – that is, support for people living in their own home, alone or with other people.
For some people this may not be an option, so Discovery also runs care homes across Somerset, regulated by the Care Quality Commission.
Whatever the format, we tailor our support for each person, based on our principles of person-centred thinking. Individuals receive support ranging from a few hours each week to multiple ratio staffing 24/7.
We promote choice and inclusion. By listening to the people and families we support, we shape a service that is as individual as they are.
We support people to access the community and develop independent living skills. We successfully support people with even the most complex needs to be part of their community, enabling them to have choice and control in their lives.
We believe that each person we support should have opportunities to develop relationships, learn new things and enjoy the activities they wish to do. Becoming part of the local community enables individuals to live an active and inclusive life.
