The people we support are at the heart of everything we do, and you will find that Discovery people are friendly, approachable and committed to the safety and well-being of your relative.
We provide person-centred support for people with learning disabilities and autism, including people who have complex needs, challenging behaviour and profound and multiple learning disabilities.
We might provide support to your relative 24/7 or perhaps for just a few hours each week, in supported living, in a care home or as day support. We might support him or her to enter employment or provide regular short breaks. You can find our more about the range of support we offer here.
However we support your relative, you’ll find that everything we do is personalised to the him or her. We believe everyone has the right to choice and control over their life. By using a number of person-centred approaches, we create support plans that focus on achieving the outcomes the people we support want, increasing people’s confidence and independence. We aim for everyone we support to lead an ordinary life as an equal citizen.
Our support works best when it is delivered as a true partnership with families. Your lived experience of how best to support your relative is priceless. We will ask for and welcome your involvement – whether in recruiting your loved one’s support team, in care reviews, or in guiding our day-to-day support. We will regularly ask you for your opinions through a mix of individual conversations and surveys, and strive to act on these. We hope we will be able to inspire you, too, with the ambition that we will show for your relative. And we hope you will find the time to compliment your relative’s staff team for a job well done; compliments, delivered through our ‘Everyday Heroes’ programme, are hugely motivating to colleagues. Equally, if you need to raise concerns then you’ll find we have a robust process for handling these.
Here, you can find a range of factsheets that we hope you will find useful to tell you more about how we hope to work alongside you.
